How to apply for Missouri Medicaid

Apply online through the Missouri DSS benefits portal — available 24/7

Missouri's Family Support Division accepts online applications at mydssapp.mo.gov. You can also apply by phone, by mail, or in person at a local Family Support Division Resource Center.

How to apply for MO HealthNet in Missouri

The Missouri Department of Social Services Family Support Division (FSD) handles MO HealthNet applications. Per MO HealthNet's apply page, there are four ways to submit:

Online — Missouri DSS portal

Apply at mydssapp.mo.gov. Create an account to submit your application, check status, and report changes. Same portal accepts applications for food assistance and other DSS programs.

By phone — 855-373-9994

Call 855-373-9994 to apply by phone or speak with a Family Support Division representative. Translation services are available — call 1-855-373-4636 and ask for an interpreter. TTY: 1-800-735-2966.

In person — FSD Resource Center

Walk into any local Family Support Division Resource Center. Staff can help complete the application, verify documents, and answer questions. Find your nearest office at dss.mo.gov/dss_map.

By mail or fax

Download the paper application (IM-1SSL) from dssmanuals.mo.gov. Mail to: Family Support Division, P.O. Box 2700, Jefferson City, MO 65102. Or fax to 573-526-9400. Keep a copy of everything you submit.

Who needs a supplemental form

Most applicants submit just the standard application form (IM-1SSL). However, you must also complete the Supplemental Form (IM-1ABDS) if any of the following apply to you or a household member:

  • Age 65 or older
  • Blind or disabled
  • Receiving Social Security benefits (any type)
  • Living in a nursing facility or other medical institution
  • Have Medicare or VA healthcare coverage

This supplemental form is required because these categories use a different eligibility methodology (non-MAGI) with income and asset tests that differ from the standard MAGI-based MO HealthNet rules. The form is available at dssmanuals.mo.gov or at any FSD Resource Center.

How long MO HealthNet takes to process

Per the MO HealthNet Division's application page, Missouri processes most applications within 45 days under the federal standard at 42 CFR 435.912. If a disability determination is required, the timeline can extend to 90 days.

If you do not receive any notice from the Family Support Division within 45 days, contact your local FSD Resource Center. If approved, you will receive a MO HealthNet ID card and information about your coverage type in the mail.

Upload documents online at mydssupload.mo.gov — no fax or mail needed

After submitting your application, any supporting documents can be uploaded directly at mydssupload.mo.gov. This is faster than mailing or faxing, and creates a record that your documents were received. You can upload pay stubs, ID documents, immigration paperwork, or anything else FSD requests.

What documents you'll need

Gather these before starting your application. Having them ready prevents delays caused by missing information requests, which can add weeks to the review.

  • Proof of identity — driver's license, state ID, passport, or birth certificate
  • Proof of residency in Missouri — utility bill, lease, or official mail with your address
  • Social Security numbers for all household members applying
  • Proof of income for the past 30 days — pay stubs, employer letter, or benefit award letters
  • Tax filing information if self-employed — prior year return is typically acceptable
  • Immigration documents if applicable — green card, visa, or I-94 arrival/departure record
  • Health insurance information if you currently have coverage through an employer or other source

Not every document is required for every applicant. The application will specify what MO HealthNet (Missouri Medicaid) needs based on your household composition.

What to expect after you apply

Under 42 CFR 435.912, states must process most Medicaid applications within 45 days of receipt. Applications based on disability take up to 90 days. If Missouri hasn't issued a decision by those deadlines, the agency must notify you in writing with the reason for delay.

Medicaid coverage typically starts on the first day of the month in which you applied, assuming you're determined eligible. In some cases — particularly for pregnant women — retroactive coverage going back up to three months may apply if you received qualifying medical services during that period.

Keep your contact information updated while your application is pending. A notice sent to an old address counts as received.

If your application is denied

A denial notice must state the specific reason and your right to appeal. You have the right to request a fair hearing — typically within 90 days of the denial notice — where you can present evidence and contest the decision before an impartial hearing officer.

Common denial reasons include income above the limit, failure to verify documents within the required timeframe, or a missing signature. Many denials can be resolved by reapplying with the correct documentation.
