The enrollee must be a U.S. citizen or legal alien and Utah resident.


Documents Needed For Utah Medicaid Application

  • Social security number and birth date of all applicants
  • Proof of income
  • Proof you live in Utah
  • Proof of pregnancy (if you are pregnant)
  • Proof of Citizenship, status as U.S. National, or legal Immigration Status
  • If you had prior insurance, have the proof with you to fill out the online application
